You can find the Dectus Medallion (Right) in Elden Ring in the region of Caelid at Fort Faroth. The Caelid region is to the east of Limgrave, the starting area of the game. Fort Faroth can be found just south of the Minor Erdtree in Dragonbarrow. This is a dangerous area, and players should approach with caution, especially if they are low level.
To get the Medallion, players will need to enter Fort Faroth and travel to the back-right area of the structure. There will be a ladder that leads to an upper platform. On this platform is a chest containing the Dectus Medallion (Right). Fort Faroth is full of Harpy-like enemies with a few high-level foes as well, and getting through can be tough. Players do not have to defeat these enemies to get to the Medallion's chest and simply run through the ladder.
What do the Dectus Medallions do?
The Dectus Medallion (Right) will work in random with the Dectus Medallion (Left) and, when taken to the Great Lift of Dectus, can be used to activate transportation to a new area of the game. The Great Lift of Dectus can be found north of Liurnia and transports players to the Atlus Plateau. This is a requirement for players hoping to beat the game's story.

Players may hear NPCs talking about the Great Lift of Dectus and how to activate it as they travel around Elden Ring's world, but their dialogue is little more than clues to the actual solution.
